Attestation of a document other than the power of attorney

Requirements for Life Certificate Attestation:

1. In-Person Attestation at the Embassy:

– Visit the Embassy in person with your original and valid CNIC/NICOP.

– Obtain the Life Certificate form from the counter, complete it, and provide your signature and thumb impression.

– The designated staff member will attest the form on your behalf.

2. Attestation by Mail:

– Download the Life Certificate form from the Embassy’s official website: embassyofpakistanusa.org.

– Complete the form, sign it, and add your thumb impression.

– Have the form notarized by a Notary Public.

– Mail the notarized form along with copy of your valid CNIC/NICOP to the Embassy along with a self-addressed USPS return envelope.

Note: Life Certificate attestation is free of charge.

Attestation of Affidavits, Wakalatnama, Undertakings, etc.:

i. Visit the Embassy in person with your original Pakistani CNIC or NICOP.

ii. Sign the document and place your thumb impression in the presence of the Embassy Officer.

iii. The attestation fee is US $32 per document (if paid through the card machine) or US $30 per document (if paid by cheque made payable to the Embassy).

Attestation of a Power of Attorney

Procedure of Attestation of Power of Attorney/Affidavits:

Attestation of Power of Attorney (POA)

1. Requirements for In Person application at the Embassy:
– In person presence of the Executor
– Printed text of the Power of Attorney
– Executor’s original valid Pakistani Passport/NICOP with photocopies
– Copy of CNIC/NICOP of the Attorney (recipient)
– Names, signatures, and CNIC numbers of two witnesses (not required to appear in person)
– Guardianship certificate (if the Executor is a minor)
– Two recent passport-size photos of the Executor

Fee: US $32 (card) or US $30 (cheque payable to the Embassy)

Online Power of Attorney (via poa.nadra.gov.pk)

– Create an account using email and verify via OTP
– Upload the signed POA text with photo, CNIC/NICOP copies of Executor, Attorney, and witnesses
– Upload biometrics on the fingerprint form
– Pay fee online by credit card and await biometric verification
– Attend a scheduled video interview with the Consular Officer
– After approval, NADRA emails a “Final Consent” request
– Upon confirmation, the applicant receives an approval PDF to attach with the signed POA and send to the Attorney in Pakistan

Attestation of Driving License

In case of driving license, Embassy would attest the letter, issued by the driving license issuing authority, conforming the particulars of the holder of driving license. The letter should first be attested by the Ministry of Foreign Affairs, Islamabad or its Camp offices in the provincial Capitals.

Important note: On required photocopies
  • Please read the requirements for photocopies for each type of application and bring/send the required number of copies with the application.
  • For the convenience of the applicants, a copier machine at the Consular Section is available, which takes either 25 cents (quarters) coins or one dollar bills only.
